Social Worker, MSW- SCS Home Health

$5,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.

Responsible for Spokane County, Washington

Schedule: Full time, Day shift

The Medical Social Worker provides community resource information and short term counseling services to patients in their homes according to the physician's plan of treatment. Quality of care is provided in accordance with the mission of PVNA Home Health Care Services.

Required Qualifications:

• Master's Degree: Social Work degree from a school of a social work accredited by the Council on Social Work Education.
• Upon hire: Washington Advanced Social Worker License Or
• Upon hire: Washington Advanced Social Worker Associate License Or
• Upon hire: Washington Clinical Independent Social Worker License Or
• Upon hire: Washington Clinical Independent Social Worker Associate License
• Within 30 days of hire: National Provider Identifier (NPI) number
• One year: Social work experience in a health care setting.

Preferred Qualifications:

• Upon hire: Certification by the Academy of Certified Social Workers.
• Two years: Two (2) to five (5) years experience in health care setting.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
