This is an intermittent position with a flexible schedule averaging 10-12 hours per week, and starts at $20.11/hr.

What you'll be doing

The Peer Navigator assists clients participating in HIV Medical Case Management Services. They offer a unique personal perspective and provides coaching and emotional support to clients living with HIV or AIDS, who may need assistance in connecting to and managing medical, support services, and case management appointments. Additionally, the Peer Navigator works closely with HIV Medical Case Managers, HIV Providers, and other community providers to provide individualized client-centered services over a short period of time. The Peer Navigator works directly with HIV positive clients as part of a collaboration with HIV Medical Case Management services with the goal of client outcome improvement. The Peer Navigator is expected to have life experiences which enable him or her to provide coaching to clients living with HIV to help them overcome obstacles to living a healthy life with HIV. The Peer Navigator helps people living with HIV meet their immediate needs, with special focus on maintaining connection with health care and by taking medications as prescribed to reduce viral load to undetectable.

Some Essential Functions Include:

  • Works with Locating Out of Care and Department of Health to locate clients that are lost to care.
  • Conducts outreach to clients or to re-engage clients who have not been coming in for services or who have missed medical appointments.
  • Reminds clients about health and social service appointments and accompanies them to health and social service appointments when needed.
  • Participates in planning and execution of client attended events such as: Strength for the Journey, Pride, HAAB, AIDS Walk, World AIDS Day.

What we need from you

Required Qualifications:

  • High School Graduate or GED.
  • One year of relevant experience.
  • Valid motor vehicle operator's license and insurance.

Preferred Qualifications:
  • Experience in navigating ongoing medical care for individuals living with HIV/AIDS.
  • Personal experience and/or knowledge about people living with HIV/AIDS.
